City of Hope’s Robert J. Morgan, Jr., MD, FACP, introduces City of Hope’s Continuing Medical Education e-Learning series, “Advances in the Prevention, Diagnosis and Treatment of Breast and Gynecologic Cancers”. Breast cancer remains the most frequently diagnosed solid tumor and second leading cause of cancer death in US women. If these rates continue, a female child born today will have about a 1 in 8 chance of developing breast cancer at some point during her life. Greater public awareness of signs and symptoms have increased early diagnosis, breast cancer treatment is still dependent on clinical factors such as stage, histology, HER-2 and hormonal status. It is estimated that 20 million Americans may have the human papillomavirus (HPV). Yet more than 76 percent of US women have never heard of this sexually transmitted virus which causes nearly all cervical cancers. Women who are not diagnosed with cervical cancer through screening typically present with advanced disease. In 2010, the NCI placed the number of newly diagnosed cases of cervical cancer at 12000 with related deaths at 4000. The scientists and clinicians at City of Hope have a long history of developing and advancing new techniques to screen for, diagnose and treat gynecologic and breast cancers. If after you’ve done a self-diagnosis at home for your existing symptoms, get in touch with a medical physician for proper diagnosis. Most women will make an annual appointment with their medical health care physician just to put their mind at ease.

There are some women who choose not to go through mammogram. Perhaps it is a matter of preference and the history of other women in the family. However, even the health insurance provider will recommend an annual mammogram in the event that a tumor is developing.

The best time to do this is while the tumor is still very small that it can just be removed easily without posing threat to the patient. The actual process from start to finish will incorporate your medical physician and your health insurance provider discussing the terms of what you will need.

This will also incorporate all the health care incidental testing that is yet to be taken and the possibilities of x-rays if necessary. The whole process requires a lot of patience from you. Although the exact causes of breast cancer have yet to be identified, the risk increases gradually with age and for certain people there may be a family hereditary (genetic) component. Other risk factors may include a long menstrual cycle, early onset of menses, or late menopause; first pregnancy after age 31 ; a high-fat diet; endometrial or ovarian cancer; radiation exposure; estrogen therapy; antihypertensive therapy; alcohol and tobacco use; and preexisting fibrocystic disease. The recent discovery of the breast cancer gene BRCA 1 confirms the theory that the disease can be inherited from either the mother or the father. About half of all breast cancers develop in the upper outer quadrant. Growth rates vary. Theoretically, slow-growing breast cancer may take up to 8 years to become palpable at 3/8 “(1 cm). It spreads by way of the lymphatic system and the bloodstream through the right side of the heart to the lungs and to the other breast, chest wall, liver, bone, and brain. Fine-needle aspiration and excisional biopsy provide cells for histologic examination to confirm the diagnosis. Ultrasonography can distinguish between a fluid­filled cyst and a solid mass. Chest X-rays can pinpoint metastases in the chest. Scans of the bone, brain, liver, and other organs can detect distant metastases. Laboratory tests, such as alkaline phosphatase levels and liver function, can uncover distant metastases. Hormonal receptor assay can determine whether the tumor is estrogen or progesterone-dependent. This test guides decisions to use therapy that blocks the action of the estrogen hormone that supports tumor growth. Treatment The choice of treatment usually depends on the stage and type of disease, woman’s age and menopausal status, and disfiguring effects of surgery. Therapy may include any combination of surgery, radiation, chemotherapy, and hormone therapy. Surgery includes lumpectomy, partial mastectomy, simple or total mastectomy, modified radical mastectomy, and radical mastectomy. Lumpectomy. (say: lum- pek -tuh-mee), which removes the cancerous tumor from the breast. A woman usually has this surgery when the cancer is found early and when the lump is small and in only one part of the breast. Partial mastectomy (also known as segmental mastectomy or quadrantectomy) removes one-quarter or more of the breast. Simple or total mastectomy is the removal of the breast but not the lymph nodes or pectoral muscles. Modified radical mastectomy is the removal of the breast and some of the axillary lymph nodes. Radical mastectomy is the removal of the breast, pectoralis major and minor, and axillary lymph nodes. The use of this surgery has declined. Before or after tumor removal, primary radiation therapy may be effective for a patient who has a small tumor in early stages without distant metastases. Radiation therapy can also prevent or treat local recurrence. Preoperative breast irradiation also helps to sterilize the field, making the tumor more manageable surgically, especially in inflammatory breast cancer. Various cytotoxic drug combinations may be administered either as adjuvant therapy or as primary therapy. The patient may base her decision to undergo chemotherapy on several factors, including the cancer’s stage and hormonal receptor assay results. Chemotherapy commonly relies on a combination of drugs, such as cyclophosphamide, fluorouracil, methotrexate, doxorubicin, vincristine, paclitaxel, and prednisone. A typical regimen is cyclophosphamide, methotrexate, and fluorouracil; it’s used in premenopausal and postmenopausal women. Hormonal therapy lowers levels of estrogen and other hormones suspected of nourishing breast cancer cells. For example, antiestrogen therapy (specifically tamoxifen, which is most effective against tumors identified as estrogen receptor-positive) is used in postmenopausal women. Breast cancer patients may also receive estrogen, progesterone, androgen, or antiandrogen aminoglutethimide therapy. The success of these therapies provides growing evidence that breast cancer is systemic, not local, and has led to decline in ablation surgery. Prevention Doctors and scientists are working on finding cures for all types of breast cancer. They are researching new medicines that may even help prevent the disease. But in the meantime, it’s important for women to catch the disease early. Regular mammograms – together with monthly breast self-exams – are the best ways for women to protect themselves. You may want to ask the women you care about if they are taking these important steps.

Symptoms and Complications
The appearance of a lump or mass in the breast is found 9 times out of 10 during a self examination. There is usually no pain, but the mass may cause an unusual sensation in the place where it is located.

When a tumour appears in the milk ducts, bleeding can occur from the nipple. The size or shape of the breast may change. In addition, the nipple may enter into or skin may shrink, causing the formation of a dimple. The appearance of a lump or swelling in the armpit, or a redness or swelling of the breast are other symptoms.

While these might be early signs of breast cancer but may also indicate another non-cancerous. In fact, about 8 out of 10 are not cancerous. It should however consult a doctor to pinpoint the exact cause of the hump.

The most serious complication of breast cancer metastasis. Metastases occur when some cells from a tumour break off and travel to other parts of the body via the blood or lymphatic vessels to invade tissue in locations distant sometimes. When metastases are formed from cells of breast cancer, they are most often in lymph nodes, lungs, liver, bones, brain and skin. Several years may pass – even after the diagnosis and treatment of breast cancer – before the cancer that has spread from the primary tumour.

When metastases are discovered, the cancer has probably already spread to other places, even if these tumours are not detected.

Diagnosis
It must be checked by your doctor a suspicious mass in your breast. Besides a complete physical examination, the doctor will also perform a mammogram, which is to say at a low dose X-rays may show abnormalities in the breast. An ultrasound can sometimes be performed to determine if the lump is a fluid-filled cyst or a solid mass. A biopsy may be necessary even if the lump tactile does not appear on the mammogram.

Fluid may be collected in a cyst with a needle to determine if it contains cancer cells. A solid mass should be analyzed under a microscope. To this end, we must perform a needle biopsy, a core biopsy or surgical biopsy.

In the case of needle biopsy, a thin needle is inserted into the lump to extract cells. Core biopsy involves removing a sample of breast tissue and cells to examine. The biopsy involves removing the entire mass and a certain amount of healthy tissue nearby. This procedure is performed under local or general anaesthesia.

Cancer cells may be subjected to examination to detect the presence of oestrogen receptors or progesterone. In addition, we may conduct further tests – including radiography, bone scan, ultrasound, computed tomography or magnetic resonance imaging – to see if the cancer has spread to other organs or tissues.

The Institute of Medicine conservatively estimates that medical errors are the 8th leading cause of death in the United States. Medical errors could be prevented. In addition to lives that have been changed or lost, medical errors cost the country approximately $17 billion per year.

One area that affects thousands of women and the ones they love every day is breast cancer. When a health care provider fails to properly diagnose a patient who has breast cancer the results can be devastating.

A possible reason that the failure to properly diagnose – or to misdiagnose – breast cancer is so common is that the medical community for the most part has the preconceived notion that the only older women are at risk and that these women also have a family history of breast cancer.

The Physicians Insurers Association of America did a study and found that the three main reasons that doctors misdiagnose breast cancer are:

1. Doctors do not take women seriously when these women describe the symptoms of breast cancer.

2. Even if the patient says that she has felt a lump many doctors still heavily rely on negative mammogram results.

3. Physicians place too much emphasis on a woman’s age as a factor for predicting breast cancer.

What is the Best Way to Detect Breast Cancer?

The key to early detection of breast cancer is to regularly test for it. If you can spot the signs early you will greatly improve your chances for a total recovery.

The two best ways to test for breast cancer are by having regular mammograms and by doing a thorough self-exam each and every month.

To put things into perspective about how important early detection is, let’s take a look at the 5-year survival rate for people with breast cancer who received the appropriate treatments. The best indicator of the prognosis is based on the clinical stage of breast cancer.

- Stage 0 – 100%

- Stage 1 – 100%

- Stage II – 86%

- Stage III – 57%

- Stage IV – 20%

If cancer is not diagnosed in time it can greatly reduce the chance of recovery.

When you visit your doctor keep in mind that in terms of cost and frequency the delay in the diagnosis of breast cancer is one of the leading causes of malpractice claims in the United States.
