CME: Advances in the Prevention, Diagnosis and Treatment of Breast and Gynecologic Cancers
City of Hope’s Robert J. Morgan, Jr., MD, FACP, introduces City of Hope’s Continuing Medical Education e-Learning series, “Advances in the Prevention, Diagnosis and Treatment of Breast and Gynecologic Cancers”. Breast cancer remains the most frequently diagnosed solid tumor and second leading cause of cancer death in US women. If these rates continue, a female child born today will have about a 1 in 8 chance of developing breast cancer at some point during her life. Greater public awareness of signs and symptoms have increased early diagnosis, breast cancer treatment is still dependent on clinical factors such as stage, histology, HER-2 and hormonal status. It is estimated that 20 million Americans may have the human papillomavirus (HPV). Yet more than 76 percent of US women have never heard of this sexually transmitted virus which causes nearly all cervical cancers. Women who are not diagnosed with cervical cancer through screening typically present with advanced disease. In 2010, the NCI placed the number of newly diagnosed cases of cervical cancer at 12000 with related deaths at 4000. The scientists and clinicians at City of Hope have a long history of developing and advancing new techniques to screen for, diagnose and treat gynecologic and breast cancers. With the goal of contributing to the improvement of quality of life and overall outcomes for these patient populations, these activity has been designed by these experts, representing multiple disciplines, to profile …

Stomach cancer is a result of a malignant cells in the lining of the stomach. Ninety percent more of stomach cancers are adenocarcinomas, arising from cells lining the stomach wall; lymphomas account for to three to seven percent of stomach cancers. For some unknown reasons, incidence of stomach cancer has decreased in the United States in the last 50 years; dietary factors and the refrigeration of foods are thought to play roles in the decline.
Causes and Symptoms
The cause of stomach tumors is unknown but environmental factors appear to play a role. Geographic areas such as the United States often have a low incidence of stomach cancer. Consuming foods containing nitrates or nitrites (chemicals commonly used in processed foods as preservatives) increases stomach cancer risk. Smoked foods are believed to pose the highest risk and also those who consume less on vegetables and fruits are at risk. Surgical removal of some part of the stomach (gastrectomy), chronic inflammation of the stomach lining (gastritis), and pernicious anemia increases the odds of developing stomach cancer. Hereditary, the disease might be passed down to a relative: for reasons that are unknown. People with type-A blood has a higher incidence of stomach cancer. Infection with the bacterium helicobacter pylori has also been associated with gastric cancer, particularly in those infected at young age. A type of lymphoma called “MALT” is linked to infection with the bacterium helicobacter pylori.
List of Stomach Cancer Symptoms:
No symptoms in most cases of small, surgically curable stomach cancers.
Stomach discomfort or having pain after meals that is not relieved by soothing foods or antacids.
Loss of appetite and weight.
Vomiting and Nausea
Gas, heartburn, regurgitation, and belching.
Bloating or abdominal fullness
Swallowing difficulty
Weakness and fatigue
Anemia due to gastrointestinal bleeding
Black, tarry stools (due to bleeding)
One of the most effective natural and alternative stomach cancer treatments is the self-administered oxygen therapy on which almost of the European doctors recommend. Further studies confirm that microbes, harmful bacteria’s and viruses such as stomach cancer, heart diseases and even aids, do not cause the disease by themselves. But when there is oxygen depletion in the body this said ailments spreads through out the natural habitat of the human body. When oxygen depletion occurs our body becomes acidic, which makes our own immune system weak and can be easily be taken down by all kinds of viruses and reproducing more harmful bacteria’s respectively. Doctor’s conclude and recommend the first line of defense is to make our system highly oxygenated. Explore and study more of this effective therapy.

Diabetes Insipidus is a rare disorder resulting from a failure to produce sufficient amounts of vasopressin which is also known as antidiuretic hormone (ADH). Vasopressin is produced by the hypothalamus and is secreted by the pituitary gland which helps the kidneys to reabsorb water and maintain proper fluid balance. When the pituitary gland fails to produce enough ADH, water is not conserved but simply passed through the kidneys and then excreted (typically in large amounts).

Prevention is better than cure.
The report proves that cancer prevention is not as difficult as many may believe.
Research shows that a large percentage of cancer-related deaths may be – are directly linked to lifestyle choices such as smoking, drinking, and a lack of exercise. But the majority of the reason is, due to an unhealthy diet.
The research shows that only 5-10% of all cancer cases can be attributed to genetic defects, whereas the remaining 90-95% of all cancers is due to identified factors, and thus are potentially preventable.
30% are due to tobacco use, and as much as 35- 50% are due to foods (fried foods, red meat). About 15-20% are due to infections, and the remaining percentage are due to other factors like radiation, stress, physical activity, environmental pollutants etc.
How Cancer starts:
There are over 100 different types of cancer, and each is classified by the type of cell that is initially affected.
Our bodies are made up of millions of living cells, grouped together to form tissues and organs such as muscles and bones, the lungs, Heart, liver, colon and so on.
The structure and function of each one of the cells in the human body is like a tiny factory providing essential, life-sustaining support.
In normal healthy adult organs cells have a genetic code of conduct (known as DNA), which dictates when they divide, grow, maintain or perform their specific jobs.
Normally, our cells obey these orders and we remain healthy. But sometimes Due to the interference of some oxidized complex chemicals (known as free radicals), the instructions get mixed up, the normal cells lose their code of conduct and this imbalance eventually leads to an overgrowth of a mass of abnormal cells.
As a result cancer begins to form.
Cancer cells accumulate and compete for nutrients, space, and territory with normal healthy cells. Start stealing food from normal cells and feed the abnormal cells which have no function.
Therefore the healthy cells start dying and Cause damage in the organs.
Carcinogens are substances that promote the development of cancerous cells. They may come from foods, from the air, or even from within the body.
When the person’s immune system is strong most carcinogens are neutralized before damage can occur.
Some vitamins, minerals and photochemical in plant foods are known to be Antioxidant, could interact in ways that boost the immune system in their individual anti-cancer effects.
In many cases, healthy living reduces the risk of cancer by reducing our exposure to things that damage DNA. These include:
Cigarette
Cigarette smoke contains 4000 chemicals, out of which 250 known harmful chemicals and more than 50 have been found to cause cancer in lung, esophagus, larynx (voice box), mouth, throat, kidney, bladder, pancreas, stomach, and cervix.
Alcohol
Drinking Alcohol can initiate cancer by damaging the cells of the sensitive tissues of the upper-respiratory tract and liver.
Choose not to drink alcohol, or choose to drink them only in moderation. That’s no more than 2 drinks per day for men and 1 for women.
1 drink = 1 bottle or can of beer (12 Oz)
1 small glass of wine (5 Oz)
Obesity and over consumption of high-calorie food
Obesity and physical inactivity may account for 25 to 30 percent of several major cancers-colon, breast, kidney.
Do not use plastic container in microwave.
Combination of high heat and plastic releases a harmful cancer causing chemical. into the food which causes breast cancer. Use glass, Corning ware or ceramic container.
There is evidence that limiting the consumption of some foods could prevent cancer.
Cancer-promoting foods are typically rich in refined sugars, processed carbohydrates, preservatives and by-products of deep-frying.
Food sources high in refined sugars such as sweetened beverages, sodas and many juices can increase cancer.
Doughnuts are the completion of three major classes of cancer causing foods; they are deep-fried, contain high levels of sugars and are based on heavily refined carbohydrates. The combination of by-products from the frying process together with heavy amounts of sugar and processed flour make doughnuts particularly risky to those hoping to reduce cancer risk.
Fried potato products such as French fries, potato chips and hash browns are also high on the list of cancer-causing food which is the by-product of processed carbohydrates and deep- frying process.
Hot dogs and other preserved, salted meats such as bacon or bologna should be consumed in moderation or avoided.
Such meats are dangerous. Because they have some harmful chemicals are into the mix as preservatives.
Burned Meats
When organic plant or animal matter is burned, a number of toxic, mutagen by-products are formed. Many of these by-products are exactly the same as those produced in the burning of tobacco that accounts for the toxic and cancer promoting effects of cigarettes. According to the National Cancer Institute, cancers of the colon, stomach and other digestive system malignancies are associated with high intake of charred or burned meats. While this effect has not yet been rigorously established for other burned foods such as burnt toast, the same effect is possible.
Minimize your exposure to environmental toxins
It’s impossible to avoid exposure to environmental toxins entirely, but you can strive to avoid the following everyday toxins through simple lifestyle choices:
Heavy metals – found in treated wood, antiperspirants, vaccines, and factory farmed fish
Dioxins – found in fat of animals that are factory farmed
Pesticides – non-organic fruits and vegetables, factory farmed meats, and bug repellents
It’s impossible to avoid exposure of some toxins entirely through simple lifestyle choices.
Some toxic chemicals are in every aspects of our life. Such as
Ingredients in your cosmetics, Personal Care Products, home-care products, baby-care products, laundry care products and so on.
Bis-phenol A, or BPA is a toxic chemical that is presently perfectly legal for use in food and beverage containers, mainly canned food. BPA is used as epoxy resin that lines metal food cans and is also found in plastic food storage containers, baby bottles, and water bottles. Soups, beans, infant formula, and other food packages may leach BPA from the can lining.
Cancer causing chemicals, health destroying compounds contaminate air, food, water supply in the form of chlorine, synthetic drugs, formaldehyde, herbicides and pesticides.
Avoiding cigarettes, minimizing alcohol, and getting regular exercise are a great start to an anti-cancer lifestyle.
But to best support your health, you need to look at your eating habits.
You may be eating many foods that fuel cancer, while neglecting the powerful foods which are full of nutrients that can protect you.
If you change your diet and behaviors, you can minimize your risk of disease and possibly even stop cancer in its tracks.
Foods high in Beta-carotene, vitamin A, vitamin E, selenium and vitamin C significantly increase elimination of toxins and free radicals known to cause cellular changes that are the root of cancer Furthermore, the addition of pro biotic is associated with improved immune function by re balancing the natural intestinal bacteria.

Lifestyle Changes Can Prevent Cancer
Research has shown that as many as 40% of all types of cancer in the world could be prevented by changing your diet, getting adequate exercise, and maintaining a healthy weight. Since cigarette smoking causes 30% of all other cancers worldwide, lifestyle changes including quitting smoking could prevent as many as 70% of all incidence of cancer on a global scale. Moreover, the incidence of heart disease, stroke, diabetes, high cholesterol, high blood pressure, arthritis and even hypertension, just to name a few, could be prevented by some simple changes in your daily life. Serious commitments to make these adjustments would save a lot of heartache, misery and premature deaths and would simultaneously put an end to the health care debate in this country.
Your Diet Matters
Your diet is the most important change you can make in relation to cancer prevention. Research shows that populations which consume 5 to 6 servings of fruits and vegetables daily have a far lower rate of many types of cancers. The antioxidants found in these living foods have the ability to knock out cancer before it starts. Fruits and vegetables are loaded with the essential cancer fighting vitamins, A, C, and E as well as beta-carotene. All this together with the high fiber content make it hard for a poor cancer cell to find any real estate in which to thrive.
Most of us have heard that tomato products serve as powerful prostate cancer preventitives in men, but don’t forget the benefits of soy products, green tea, and selenium rich foods as super colon cancer inhibitors.
The key is a low fat diet which fruits and vegetables provide perfectly. Certainly animal fat must be avoided at all costs if you want to have the best chance of living your life cancer free. It should not be a secret at this late date that a diet high in fiber, fruits, vegetables, nuts, grains, and legumes is just what the doctor ordered in the fight against cancer, heart disease and a host of other fearful infirmities. If you or someone you know is not yet aware of the massive amounts of research that prove that your diet matters, you should make every effort to become informed because it truly is a matter of life and death.
Some of the American Cancer Societies recommendations to reduce the risk of cancer are:
-Stick to a mostly plant-based diet and get a minimum of 5 servings of fruits and vegetables each day.
-consume whole grains as opposed to refined grains or processed sugar.
-Stay away from high fat foods especially animal flesh and their by-products.
-Get adequate physical exercise.
-Limit alcohol consumption.
-Don’t smoke… Period.
Breast Cancer
Since this is breast cancer awareness month I should add a couple of do’s to the above list just for women:
-Do add walnuts to your shopping cart.
-Do get flax-seed meal into your diet. This is truly a wonder food.
-Do get at least 1000 IU of vitamin D or more each day. If you are timid regarding sun exposure then it is imperative that you take a good supplement.
-Do drink plenty of green tea.
-Do eat plenty of fiber
-Do get plenty of folic acid.
-Do add soy to your diet. There is not one conclusive report that suggests soy is responsible for breast cancer rates going up, but there is ample documented evidence that it in fact it substantially lowers breast cancer rates especially after long term consumption.
-Do drink plenty of water.
-Do stay active and exercise.
It’s Simple
Nothing that has been written in this article is an unbearable task to accomplish. Granted, triple by-pass surgery might be just a bit easier since all you have to do during the operation is lay there, but certainly if you do live to tell about it you will be instructed to implement these lifestyle changes as soon as you are able to walk on your own.
As a former heavy smoker I will admit that cigarettes can be very difficult to “just say no” to, but which one of the other healthy suggestions mentioned above is so monumental? Even smoking can be dealt with if you just realize that you are going to have a pretty rough time for the first 10 days. After that it will get progressively easier and will be a non issue after 30 days. Certainly you can do anything you really put your mind to for a mere 10 days and trust me the cravings will significantly and progressively diminish after this period. Relax, you can do this!
Please don’t wait until your loved ones are at your hospital bedside doing another night long vigil of crying and praying, before you have a change of heart and decide to make some modifications in the way you do things. It is not a matter of all or nothing either. Take some baby steps if you have to, but just start moving in the right direction. Like the tortoise and the hare, you will reach your goal if you keep plodding along. I implore you dear reader to begin to practice some or all of these lifestyle changes before it is too late. If you don’t have the motivation to do it for yourself, then please do it for the ones who love you. They will be glad you did…

I am passionate about health issues, and the state of the health of our wonderful America. I believe the American Diet is literally killing us and I believe that lobby money is the reason that we have been brainwashed into the shift from a plant based diet to an animal based diet. I believe Americans are suffering from a lack of truthful information concerning our diets. I enjoy writing motivational articles that will help to correct the problem regarding this lack of information and also examine the prevailing misinformation in the light of truth.

Of the many diseases that affect people these days, cancer is among the most feared. But despite a wealth of scientific data, most people remain unaware of how they can reduce their risk of developing cancer. All the evidence points to a low-fat, high-fiber diet that includes a variety of fruits, vegetables, whole grains, and beans, as being the best for cancer prevention. Not surprisingly, vegetarians, whose diets easily meet these requirements, are at the lowest risk for cancer.

The human body contains around seventy-five percent water. Knowing this, a best diet plan should incorporate foods containing enormous quantities of this fluid. Are Physicician’s Relationships To Pharmaceutical Companies Unhealthy For Their Patients? (Part One)
The New England Journal of Medicine reports that 94% of all physicians receive some sort of compensation from drug companies. These incentives may be in the form of food and beverages in the workplace, free drug samples, reimbursements for professional meetings or their continuing medical education, and direct payments for consulting, speaking, or enrolling patients in trials or studies. Such gifts and payments have a direct influence on the physician’s ability to make sound medical decisions.

Most doctors in America spend the whole of their practices treating symptoms. The emphasis is continually on treating the disease after it occurs. Nearly every penny of every dollar spent on health care in America goes towards treating illness, and almost nothing goes towards preventing the illness in the first place. Most of us have vivid memories of our mother’s admonishing us to eat our vegetables. And according to my recollection, I hated them. Free Radicals and Antioxidants
Free radicals are atoms or molecules with unpaired electrons. They are the reason for tissue damage in our bodies, many diseases, and they are the main culprits behind premature aging. These bad molecules are exceptionally volatile, so they seek to attach themselves to other molecules, which ultimately cancels their vitality and bolsters this disadvantageous process. Antioxidants on the other hand are the good molecules found in various foods and they actually put a halt to the process…

Cancer is a disease that causes cells to divide uncontrollably and even attack surrounding cells. Cancer cells are capable of spreading from the site of origin to other parts of the body via the blood and lymph system.
Cancerous cells grow and multiply to form tumors. All tumors are not strong enough to spread to other parts of the body. The ones capable of spreading are called malignant cancers. Some cancers like leukemia do not form tumors. Cancer doctors treat malignant tumors as soon as they are detected, else they can prove to be lethal.
Though the various cancer names stem from the site of origin, cancer doctors classify cancers in a different manner. Carcinoma is a cancer of skin or tissues lining or covering the internal organs. Sarcoma is the cancer of bone, fat, cartilage, blood vessels or connective tissue, whereas leukemia is the cancer of blood forming tissue. Lymphoma is the cancer that begins in the immune system and cancer that begins in the brain and spinal cord tissues is central nervous system cancer.
Causes
Smoking, though not the only cause, is definitely related to cancer of lungs, mouth, throat, bladder and kidney. Too much sun exposure and exposure to hazardous chemicals classified as carcinogens can cause melanoma, the cancer of skin and other cancers.
Radiation is also a cause of cancer. In very rare cases, radiation for one type of cancer can cause another cancer later in life. Heredity is another factor that causes cancer due to inherited genes. In very rare cases, skin cancer develops in burn scars many years after the burn injury has healed.
Prevention
Giving up tobacco and alcohol can help prevent not only certain types of cancers but many other diseases also. It is never too late to give up tobacco in all forms to reduce the risk of cancer. Skin cancer can be avoided by keeping away from strong sunlight and using adequate SPF.
Having less of red meat and processed meat is also helpful as cancer doctors have found that red meat like beef and pork and processed meat like bacon are related to cancer risks. Plan a healthy diet that keeps you fit and healthy as well as boosts immunity.
Regular screenings are also important. Women can conduct self breast examination regularly as well as get a Pap Smear test, especially if they have a family history of breast cancer, cervical cancer or ovarian cancer.
Treatment
The three main treatments available for cancer are surgery, radiation and chemotherapy. All of these are successful in removing cancerous tissues as well as eliminating the chances or recurrence, provided the cancer is detected well in time. There are some side effects of radiation and chemotherapy which are temporary and much easier to deal with than the cancer itself.
Other treatments for cancer include hormone therapy, bone marrow transplant and immunotherapy. These are generally given along with surgery to ensure faster healing.

Cancer Prevention is Not Rocket Science

Cancer prevention is not some kind of rocket science. It is easier than you may think. Cancer prevention involves changing habits, especially the bad ones. It means making significant lifestyle changes. Cancer prevention is an important and active area of public health research. Cancer Prevention and risk-reduction strategies can greatly lower the physical, emotional, and financial burden of cancer and improve the overall health of cancer survivors, including lowering the risk of the cancer coming back or the formation of a second cancer. Cancer prevention research in the Department of Foods and Nutrition has been ongoing from many years, however recently the strength of this area has grown.
Cancer
Cancer is not something that happens to someone else. Cancer is not something to pussyfoot around with. Cancer has been around as long as mankind, but only in the second half of the 20th century did the number of cancer cases explode. Cancer thrives on sugar and refined grains. Cancer grows rapidly where there is low oxygenation of cells. Observational evidence shows associations between alcohol consumption, physical inactivity, and obesity and increased incidence of certain cancers. Remember, the more underlying conditions causing cancer that you correct, the better your chances of beating it.
Prevention
Prevention offers the most cost-effective long-term strategy for the control of cancer. Prevention means avoiding the risk factors and increasing the protective factors that can be controlled so that the chance of developing cancer decreases. Prevention of cancer is always better than spending lots for its curing.
Research
Researchers have found that carotenoids in dark green leafy vegetables are effective in inhibiting the formation of breast and skin cancer tumors, and preventing lung and stomach cancer. Research shows that you can greatly reduce your risk of developing cancer by not smoking/stopping smoking, being sun smart, eating healthy and being physically active Drinking less. Research suggests that up to 35 percent of cancers are related to poor diet. According to research, only 60 percent of women follow breast cancer screening guidelines, 30 percent of men and women follow colon cancer screening recommendations, and only 40 percent of men regularly get tested for prostate cancer.
Supplements
Supplements aid your body to make it more healthy and leave it up to the body to take care of the cancer. Supplements can make up for this by giving the body a consistent supply of essential compounds. Supplements While some supplements may help, nutrients from foods prove more effective. If a doctor is not tied in and blinded by the medical/drug industry, he or she finds that a comprehensive approach to fighting cancer using powerful supplements that deal with the many different issues in cancer, works much better than only using drugs and other treatments that harm the body. Not using natural supplements to fight cancer is like being in a life and death struggle, and choosing to fight with one hand tied behind your back.
Cancer prevention does not have to be a hard task. Cancer prevention is a lifelong plan including smart choices in our day to day lives. Cancer prevention is important to having a long and healthy life, and it is something not to take lightly. Cancer prevention and early detection begins with knowing your cancer risk; then, doing something about it. Cancer prevention is a life- long commitment to a long- life of healthy living and it’s never too late to start.

Edie Falco
Edie Falco, star of both HBO shows “The Sopranos” and “OZ” was diagnosed with breast cancer when she was a young 40 years old, in 2003. She didn’t tell people at first, but instead secretly went into treatment. She didn’t reveal her cancer until after she was declared cancer free, but after that talked about openly. She was only secretive because she didn’t want to be asked constantly how she was feeling and if she was all right.
Jaclyn Smith
Jaclyn Smith whom everyone knows as Kelly Garret, one of “Charlie’s Angels” from the same-named TV show of the 1970s, was diagnosed with breast cancer when she was 56 years old in 2002. Most cases of breast cancer occur in women who are 55 years or older. Jaclyn was treated with a lumpectomy and radiation, because the disease was caught early during a routine self-exam. She works with groups like Susan G. Komen for the Cure and talks publicly about early detection and breast cancer risks.

Christina Applegate
Christina Applegate is one of the youngest and most vocal celebrities with breast cancer. She was diagnosed in 2008 when she was only 36 years old. The former star of “Married with Children” was filming her new successful hit show “Samantha Who?” when she was diagnosed. She tested positive for a gene mutation that greatly increases the risk of breast cancer. And because her mother had also been diagnosed with breast cancer more than once, making heredity just one more risk factor for the young woman, she opted to not just have the breast with the cancer removed, but to have both removed. This double mastectomy was a preventative measure to stave off any future cancer. Applegate speaks out for the available of MRI testing for women of all income levels, because it was this test that detected her cancer early and has probably saved her life by reducing her risk of the cancer spreading or recurring.
Melissa Etheridge
Rocker Melissa Etheridge is one of the most recognizable celebrities with breast cancer. She was diagnosed at age 43 in 2004. She had a lumpectomy, which removes only the lump, and then an aggressive course of chemotherapy to help prevent spread and recurrence. She caught the cancer while giving herself a self-exam in the shower, and gave a performance in 2005 at the Grammy awards while bald, after having lost her hair to the chemotherapy. Many people in her family died of cancer, so she is now an outspoken advocate of prevention and research.
Sheryl Crow
Rocker Sheryl Crow is also one of the most well known celebrities with cancer. She was diagnosed at 44 in 2006, and is outspoken about early detection. Surgery and radiation were her treatments, with no chemo because the cancer was caught in the very early stages.

Powerful Antioxidants For Breast Cancer Prevention and Healing

Do you know foreign bodies attack your body every day? These foreign bodies or molecules are known as free radicals. They are known as free radicals because they have lost an electron and thus are roaming freely about with no attachment to an atom. However, the first atom they find they will become attached to it. Once they become attached to the atom, the cell destroys what is there, or replaces the cell. When this replacement has been completed, the once healthy cell now becomes a bad cell. Then the bad cell goes after another healthy cell and converts that cell into a bad cell. In time, these groups of cells become mutated and cancerous. This is how you can get any type of cancer, including the breast cancer.
In order to reverse this process, you need to have an adequate supply of antioxidants such as fruits, vegetables, and other foods. The three most powerful antioxidants are beta-carotene, vitamin C, and vitamin E. This powerful antioxidants can heal you to prevent the breast cancer and also cure it.
Beta-carotene is what gives many fruits their appearance. It also stimulates the release of natural killer cells, which hunt down and destroy cancer cells before they can do any damage. It only takes about 15 to 30 milligrams a day to stop these free radicals and destroy them. You can find beta-carotene in cantaloupes, sweet potatoes, spinach, and Chinese cabbage.
Vitamin C is the next powerful antioxidant. According to scientific studies at the University of California, those who took at least 66 milligrams of vitamin C every day, found their bodies to be free of any foreign antibodies.
The most versatile antioxidant is vitamin E. This vitamin not only blocks free radicals, it also stops cancer by stimulating the immune system. When the immune system is engaged, it goes about its job of destroying any bad cells in the body.
The best way to get vitamin E is by eating wheat germ. Other foods that contain vitamin E are whole grains, legumes, nuts, and seeds.
The breast cancer can be cured and prevented. Just believe it. It is possible. It has been done before.
